Registration shall be at the office of Professional  Teacher 3^rd floor PRC ANNEX building Sampaloc Manila. Registrant will be required to submit the following:

1.       Duly Accomplish Oath form or Panunumpa ng Propesyonal

2.       Current Community Tax Certificate (CEDULA)

3.       2pcs 2x2 and 1x1 picture ( Colored white background with complete name tag )

4.       Two documentary stamp for oath form and board certificate

5.       Payment of 1050 Php ( 600 for initial payment and 450 for annual registration.

6.       One short brown envelop

Successful examinees should personally register  and sign in the roster of registered professional.

Oath taking will be on:

 For NCR Passers 8:30am at the Cuneta Astrodome, Pasay City

For Region 4 Passers 1:00pm at the Cuneta Astrodome, Pasay City
